We are a family owned and operated supportive homecare agency. We are happy to help with house chores, laundry, groceries, preparing meals along with offering transportation services to and from, appointments. Below you will see some of our more popular services that are offered.
There is no service or job too big or too small for Holders of Hope; from maintenance cleaning to seasonal cleaning even to hoarding clean outs- Holders of Hope has you covered. We are able to identify and address any need you have within your home, to keep you safe and healthy.
Our motto at Holders of Hope is "with a little bit of HOPE anything is possible". Our core group of management has years of experience in providing quality homecare services. We love what we do! Our dream is to extend the mindset of our motto through serving others with the intentions of making a difference and touching their lives, in the way that our lives have been touched by those we have cared for.
What we do? Our goal is to help our clients and patients remain in their home for as long and safely as possible. We can come along side of our clients, patients and their families and pick up where we are needed. Holders of Hope also offers local resources to help with family education when requested.
We love to help our fellow community members remain independent within their own homes. Sometimes this means we are just “present” when certain tasks are being completed, when the struggle is too great, we can step in temporarily to complete tasks however most of the time, this means that we help to adapt tasks for easier completion by our clients and patients.
What makes us different? We offer client focused plans, which means that we take immense pride in developing the perfect care plan to meet your and your family's needs. Our case manager can help you and your family talk about and identify your needs; she is also able to help find local resources that may be able to help with things that come up during your care.
Holders of Hope prides itself on identifying client/patient needs and pairing clients/ patients with their perfect caregiver. We do understand that not EVERY caregiver that comes into your home will be the right fit. Therefore, we do our best during the admissions process to take the time to get to know you and learn your preferences. We want you and your family to be comfortable in your home while our staff are there to help you out. We are committed to functioning on complete honesty and transparency. We understand that not one care plan fits all. Daily services can include anything from meal preparation, hygiene, cleaning, and supervision. We will take the time to get to know you and develop an individualized care plan that fits your specific needs.

Our agency has a 24 hour a day (365 days a year) on-call phone number that is ALWAYS answered by a member of management. Our team has been built on a powerful desire to serve others.

Supportive homecare encompasses much of a person's life. Our dynamic supportive homecare team has many years of experience in recognizing needs, developing and implementing our client - focused care plans. Holders of Hope offers a community resource specialist to help an individual and their family connect with local community-based programs. There are many programs that can help to off-set the cost of in-home care.
We work with the major MCOS's within Columbia, Dodge and Jefferson Counties.
An MCO, is a "Managed Care Organization". In many instances when a person is referred to their local ADRC, the ADRC will go through a series of questions with the person. Depending on the answers to the questions and eligibility of the person they may qualify for a "Family Care" program. Family Care can be described as "an innovative program that provides a full range of long-term care services, all through one flexible benefit program".
Holders of Hope proudly contracts with the following payer sources: Columbia County ADRC, Dodge County ADRC, INCLUSA, IRIS, & My Choice Wisconsin.
We also happily accept privately paying clients and patients.
The short answer is - in many situations YES! We are happy to work with family and friends who want to be involved in your care. We understand that often times family and friends have a strong desire to help their loved one with various household tasks but cannot afford to give up hours at their job to do so. We are proud to encourage family and friend involvement and help foster that important relationship.

We have a handful of extremely wonderful programs that are geared towards members of our community who are in need.
*We offer a letter writing program in which we have volunteers from the community write letters of encouragement to our clients / patients. How this works, community members call in and they are "given" a client to write to. The client's interests are shared along with other topics the client enjoys talking about (all with client consent), and then the community member will write the letter and send it to Holders of Hope and then we will send it out to the client. Holders of Hope will act as a go between in order to protect our client's information and privacy. When / if the client responds (most of them do), our caregivers will pick up the letter from the client and get it to the office, then we will get the response in the mail to the community member. A perfect circle of HOPE! Wouldn't you say? We serve many individuals who do not have family or have very limited family interaction, sometimes it is just a lovely gesture to be able to bless someone through a piece of mail that is sprinkled with HOPE.
*We offer a FREE service to patients on hospice, to have a volunteer sit with them to record their memories and / or letters to their families. Volunteers must undergo a state satisfied background check.
*We offer community members the ability to "adopt a senior". How this works, is that community members contact Holders of Hope and simply ask for "local needs"; we never hand out personal, private information- instead we can give a demographic of the person in need along with a list of needs. We have also had some very generous community members ask for a list of "wants"- this was really neat as it allowed for a few of our clients to have something they really wanted.
*We have an amazingly cool program called "Santa Stop Here". Let me tell you all about it! We serve many individuals who desire to have Santa visit them on Christmas Eve, in fact they watch out the window and wait to see the lights and listen for the sounds of the hooves on the roof! It is awesome! We have developed a program very similar to the "adopt a senior" program, the only difference is this program is geared towards the Christmas season and available to clients of ALL ages 18-999. It is not just about Santa stopping by to leave gifts, it is about the magic of Christmas and getting ready for what the "Christmas Season" is all about. We had a good handful of individuals that had no Christmas decorations this year! We were able to muster enough funds through VERY generous donations in order to provide trees, decorations and stockings for everyone without a tree (who wanted one).
